The CEDRiC team is proud to share the final CEDRIC toolkit. This toolkit outlines the process required to implement this evidence based model.

Evaluative research outcomes demonstrate the CEDRiC model, including a Nurse Practitioner Candidate at a large regional Aged Care Facility and the GEDI intervention at the local Emergency Department was successful in:

  • improving outcomes;
  • reducing admission to hospital;
  • reducing length of stay in the ED and hospital if admitted; and
  • saving money to the health service.

NB: Outcome publications pending.

Reducing admissions of older people reduces the risk of complications often experienced with hospitalisation and therefore improving care of this vulnerable group. We hope you can use this tool to begin the process of implementing all or part of these interventions in your region.
